This short Google Form collects the remaining acknowledgments and permissions BUCS needs, including emergency medical authorization, photo/video preference, TAIAO eligibility acknowledgment, participation consent, and electronic signature.

Why This Form Uses a UIL Template
BUCS uses the physician examination page from the UIL Preparticipation Physical Evaluation because it is a familiar sports physical form for Texas physicians. BUCS is not affiliated with, governed by, or a member of UIL. Information submitted to BUCS is maintained by BUCS for athlete eligibility and safety and is not submitted to or shared with UIL.
Send BUCS the Signed Physical
After the physician signs the form, email a clear PDF or photo to BUCS. Please include the athlete's full name in the subject line.
